Please provide a video lasting up to 3 minutes expressing your personality and telling us about yourself and why you should receive the scholarship.
IMPORTANT: Please upload the video (3 min max) to Dropbox, or Google Drive and paste a download link to your private video on the Submission page. DO NOT post to a video-sharing site (ie. YouTube, Vimeo, etc.).

Click on Enter Your Submission and answer all questions honestly and briefly. Add your link of your video in space allocated.

VIDEO RELEASE

By uploading your video to the application website (“User Content”), you hereby acknowledge and agree that you are granting Organic Evolution Pty Ltd, T/A DCI Education (including without limitation, its licensees, affiliates, successors and assigns) a worldwide, royalty-free, perpetual, irrevocable, sub-licensable, non-exclusive right and license to use, reproduce, publicly display, publicly perform, modify, sublicense, and distribute such user content, and incorporate it in other works, in whole or in part, in any manner and any media now known or hereafter developed.

APPLICATION RELEASE

By submitting this application, I hereby consent to the recording, use and reuse by Organic Evolution Pty Ltd, T/A DCI Education and any of their respective licensees, successors, assignees, parents, subsidiaries, or affiliated entities, and each of their respective employees, agents, representative, officers and directors (collectively “Releases”) of my voice, actions, likeness, name, appearance, biographical material, and any information contained in, derived from or obtained in connection with my application to be a participant in “Hair Heroes Scholarship” or in any materials submitted by me in connection with my application (collectively “Likeness”), as edited, altered, or modified by the Producer or by any of the other Releasees, in any and all media now known or hereafter devised, worldwide in perpetuity, in or in connection with the Program or any other work.

TERMS & CONDITIONS

You must be living in Australia within the cities nominated.

• You or any member of your family cannot be associated in any way with DCI Education and or Organic Evolution Pty Ltd.
• You cannot be a current client of DCI Education / Organic Evolution Pty Ltd.
• The Scholarship cannot be redeemed for cash or credits.
• The Scholarship cannot be transferred to anyone at all. It must be for the selected individual only.
• The Scholarship cannot be deferred to another year.
• DCI Education is not responsible for travel to and from workshop locations.
• DCI Education/ Organic Evolution Pty Ltd reserve all rights to cancel any successful applicant.
• DCI Education / Organic Evolution Pty Ltd reserve all rights to transfer any successful applicant to the Elevate Program online as an alternative if it chooses to do so.
• If DCI Education receives under 3 applicants for a state and or city, then the scholarship will not go ahead in that state or city.
